A customized Investigator Manual is created for each study, which details the specific supply information needed.

Specimen Collection Kits

Supplies for collecting specimens are provided in visit-specific kits. Labels on the side of each kit denote the visit for which the kit is to be used, as well as the kit expiration date. Bar-coded kit components are tracked and must not be switched between kits.

Supplies included in each kit include:

• Collection tubes and aliquot vials
• Refrigerated and/or frozen specimen transport bags
• Bubble bag to protect glass tubes during shipment
• Plastic transfer pipettes
  

Supply Order Form

You can order kit supplies or shipping supplies, such as shipping containers, online or by fax.

A new Supply Order Form is required for each visit-specific kit. Refer to your Investigator Manual for protocol and account numbers, and for examples of the Supply Order Form. Kits are not automatically resupplied.

Do the following:

• Submit the Supply Order Form online. Fill in all required fields completely.

OR

• Fax the hardcopy Supply Order Form to 800.441.1297. The form is included in each visit-specific kit. Use black ballpoint pen to fill in the required information. This step helps ensure accuracy, and avoids compromising specimens.

A minimum of five (5) full business days for the receipt of supplies is required for all requests, including ASAP and RUSH orders. Supplies are sent using an appropriate courier service.
